Referral forms

When you send a referral, we collect the information you provide — names, contact details, the services requested and any documents you attach — so our intake team can respond. Referrals are stored securely and are used only to follow up on the referral. Do not use this website for emergencies: call 911, or call or text 988 for mental health crisis support.

Advertising measurement

We use Google Ads to measure whether our advertising leads to referrals. When you arrive from one of our ads, Google may receive technical information about your visit (such as the pages you view and whether a referral form is submitted) so we can understand which ads are effective. We do not send Google the contents of your referral or any clinical information.

In regions that require consent, this measurement runs only if you accept it in the cookie banner, and you can change your choice at any time from the banner when it appears. Learn more at Google's Business Data Responsibility page.
